Apple Tree Medical is a vaccination provider for the Phase 1b of the COVID-19 vaccination rollout. We will be running vaccination clinics at each of our locations for eligible Phase 1b patients.

This page is designed to be a central point for you to access COVID-19 vaccination information specific to Apple Tree Medical.


1st Step

Am I elligible?

Firstly you have to check you are a Phase 1b eligible patient.
The following groups are included in the Phase 1b rollout.

  • Elderly adults aged 80 yrs and over
  • Elderly adults aged 70-79 years
  • Other health care workers
  • Aboriginal and Torres Strait Islander adults 55 years and over
  • Adults with an underlying medical condition or significant disability - People will need to provide proof of these conditions to demonstrate their eligibility from vaccination via Myhealth Record, a health professional referral if required or a declaration form.
  • Critical and high risk works including defence, police fire emergency services and meat process - Workers will need to provide proof of occupation to demonstrate their eligibility.

2nd Step

Am I ready?

We are really going to need your help rolling this out in a timely manner.

We are having to vaccinate as many people as possible over the coming months so please understand that these clinics are not able to cope with any other issues or concerns.

Please don't book in to the vaccination clinic unless you have done all the research you need to do about the vaccination, you are an eligible person and you are ready and willing to be vaccinated.

Any other issues and concerns (including and discussion about whether you should or should not have the vaccination in your specific circumstances) need to be addressed in a normal consultation with your GP prior to booking in to the clinic.

We have been provided with the AstraZeneca vaccination for distribution so focus your information gathering in that direction.

3rd Step

book your vaccination

We are only taking telephone bookings initially for vaccination clinics, we may expand to an online booking system once the clinics are established and running smoothly.

Please also book your vaccination at your usual clinic as that clinic will hold your up to date medical history which will help stream line the process

4th Step

attend your appointment

Please make sure you are at the surgery 10 or so minutes prior to the appointment time you have been given.
If you are unable to make the appointment please re-schedule your appointment as early as you can so we can fill your spot with another patient.
When you arrive you will be asked to complete a pre-prepared consent form. Please do so honestly.
You may also been asked to provide some evidence of your eligibility if it is not immediately obvious that you are eligible (such as proof of occupation)
You will see a doctor briefly to go over the consent form and then you will have the vaccination administered by either the doctor or one of our nursing team.
You will then be asked to wait for 15 minutes to ensure you do not have any adverse reaction to vaccination.
Prior to leaving the clinic we will make arrangements for your second vaccination to ensure maximum protection.


5th Step

Report any adverse events

Reporting suspected side effects associated with a COVID-19 Vaccine.
